However, due to financial considerations, the patient requests a restoration with the least expensive option of a composite restoration.

In an ideal world, this tooth would be best served with core buildup and a full coverage crown. Case 2Ī second molar has an old, leaky, previously repaired Class II restoration with recurrent radiographic decay (Figure 5). The highly filled composite not only has excellent aesthetics, but it will also achieve a high polish and demonstrate a very low wear rate over time (Figure 4). The restorative composite (GrandioSO, Voco) is placed and sculpted into its original shape and light cured.įinally, the restoration is shaped into its final anatomic shape and polished. Once rinsed and lightly dried, a universal bonding agent (Futurabond U, Voco) is applied, air dried and light cured.Ī thin layer of flowable composite (GrandioSO Light Flow, Voco) is applied to the most apical part of the proximal box to ensure that there are no voids in this caries-prone area (Figure 3). The ring is designed not only to achieve a high matrix fit, but also to provide additional proximal separation in addition to what the wedge provides (Figure 2).Ī total etching technique is used and the entire preparation is etched with 35% phosphor etching (Vococid, Voco). The Sectional Matrix is anatomical in design, helping to create a more anatomically correct proximal contour. We have had several years of excellent results using a sectional matrix system (ComposiTight, Garrison). The quadrant is isolated using the Isolite system (Zyris) to avoid any contamination during placement of the restoration.Īn essential element of the success of the restoration process is an excellent matrix system. A caries detection agent (Seek, Ultradent) is used to ensure that any cavities are removed. A conservative preparation is made with a 330 carbide bur and the decay is removed with a slow speed round bur. Case 1Ī second premolar radiographically shows decay and the distal marginal ridge has been undermined and fractured (Figure 1).
The following case studies will illustrate this proven hardware. But the longevity is also determined with an appropriate technique. These physical characteristics have contributed to the long-term clinical success of GrandioSO and its continued popularity in the market even after 10 years.Ĭertainly, the longevity of a composite resin is partly due to excellent physical properties. Its modulus of elasticity, thermal expansion and flexural strength are very similar to tooth structure. GrandioSO has an 89% filler content, creating an excellent wear rate. Since then, Voco has expanded this line of composites to include GrandioSO x-tra (bulk fill), GrandioSO Heavy Flow, GrandioSO Flow and, more recently, GrandioSO Light Flow, based on the same chemistry that led to the product’s success. In 2012, GrandioSO (Voco) was introduced to the US market.
The composite resins we have today are light years better than the materials we were using in the late 1980s and early 1990s.ĭemand from dentists and their patients has driven dental material manufacturers to continue improving composite resins to meet higher aesthetic and functional characteristics.
